Performance Improvement Program
Dear Doctor,
Based on a pilot program that showed real patient care improvement, the University of Virginia School of Medicine and CardioVillage are pleased to invite you to participate in an innovative study on the treatment of patients with Acute Coronary Syndrome (ACS). We are currently recruiting cardiologists and interventional cardiologists for Identify, the first phase of our 4i-ACS Performance Improvement Program. (Please see attached a description for more information on the Program). Use the data from this PI-CME Project to Complete ABIM’s Self-Directed PIM and Earn ABIM Maintenance of Certification points. This activity is supported by an independent medical education grant from Bristol-Myers Squibb/Sanofi Pharmaceuticals partnership.

As a participant in the Identify phase, you would be asked to review and report upon the treatment practices applied to 25 patients with ACS. A patient profile may take as little as 5 minutes to complete. A data report will be generated allowing for a self-reflection exercise comparing your individual data with those of your colleagues. In return for submitting 25 completed Patient Profiles and a practice improvement plan online, you will be entitled to receive 5 AMA PRA Category 1 CreditsTM.

To participate, simply register by clicking on the link provided below. Please note that for each additional phase you participate in, you will be eligible to receive 5 AMA PRA Category 1 CreditsTM. If you participate in all three phases, you will be eligible to receive a total of 20 AMA PRA Category 1 CreditsTM . Honorarium will be provided for involving peers in a face-to-face meeting to share your learning.
